Important Employee Information:
There are a couple of new forms all employees should be made aware of. First, the out of district travel form has been revised. Second, there is a new pre-approval form for travel that must be signed by the person in charge of the funds being used. If the form is not approved prior to travel, the travel will not be reimbursed. If there are questions about either of these, feel free to contact us. Both forms are attached for your convenience. There are two versions of each form. One is an electronic version, and the other is a paper version.
We have added a change of Name/Address/Phone form. These must be returned to Janet Abner at the Central Office.
If you wish to change your bank account, you must have a voided check or something from the bank showing your information. We will not accept written instructions from the employee.
